U.P. Home Health & Hospice is proud to announce the agency has achieved a 5 Star rating for patient surveys through the Home Health Consumer Assessment of Healthcare Providers and Systems (HHCAHPS) and 4.5 Star Rating for quality measures.
HHCAHPS surveys patients to make it easier for families to compare home health agencies. The survey collects feedback from current or recently discharged home health agency patients (or their family) about their experiences. The quality of patient care rating is determined using each home health agency’s scores on 7 individual quality measures, which are based on home health agency patient assessments and Medicare claims. HHCAHPS conducts these surveys completely independently.
