Radio Results Network has partnered with Transition Assisted Living in Gladstone to offer one deserving person a ($10,000 Grant) towards their stay at Transition Assisted Living. Please fill out all of the information below, telling us of your hardship and struggle to find the right assisted living that suits your specific needs and budget. One deserving person will be selected at the end of the month, so don’t wait! Due to overwhelming number of requests, they have extended the date to June 30th and are adding in additional grants.

*$10,000 Care Grant will be prorated monthly on a 12 month stay. Semi-finalists will be contacted by Transition Assisted Living for a phone and in-person interview. The Final Grant Recipient will be notified directly by Transition Assisted Living. Judges decisions are final.
